Erectile dysfunction, sometimes called "impotence,"
is the repeated inability to get or keep an erection firm enough
for sexual intercourse. In older men, ED usually has a physical
cause, such as disease, injury, or side effects of drugs. Any
disorder that causes injury to the nerves or impairs blood flow
in the penis has the potential to cause ED.
ED is treatable at any age, and awareness of this
fact has been growing. More men have been seeking help and returning
to normal sexual activity because of improved, successful treatments
for ED. For some men oral medications may not be an appropriate
treatment. This could be due to intolerable side effects caused
by the oral agents, or a lack of efficacy.
Erectile dysfunction treatments vary, but one
therapy that is recognized by the American Urological Association
as being safe and effective is an intracavernous injection of
multiple medications (usually referred to as Tri-Mix). A Tri-Mix
compound consists of the medications alprostadil, papaverine,
and phentolamine. The AUA states, "As monotherapy, alprostadil
is the most popular vasoactive agent; however, combination therapy
with other vasoactive drugs (Bi-Mix and Tri-Mix) can either increase
efficacy or reduce side effects" - (AUA Guideline on the
Management of Erectile Dysfunction).
